The Legend of Zelda: Ocarina of Time.

There wasn’t enough room in the image, but the glitch is actually triggered by beating Gohma and dying at the same time. It alters some of the values of the the teleport and doors so that the next room you enter is the beginning of Ganon’s castle. A more detailed view and description in the video below.
